Interspinous Spacers in the Lumbar Spine
نویسنده
چکیده
Recently, a new class of spinal devices called interspinous spacers (ISPs) has been introduced. A number of designs have been developed, which vary from non-compressible metallic spacers to compressible cushion-like devices. Following the path of other spinal devices, they are composed of materials ranging from allograft bone, titanium alloy, polyetheretherketone (PEEK), and elastomeric preparations. Though they are unique in their details, they share the common mechanical goal of effecting distraction between adjacent spinous processes, whereby they block intervertebral extension at that level. The proposed indications for ISPs include degenerative spinal stenosis, discogenic low-back pain, facet syndrome, disc herniations, and non-traumatic instability.

Lumbar interspinous spacers: a systematic review of clinical and biomechanical evidence.
STUDY DESIGN Systematic review. OBJECTIVE To evaluate the current biomechanical and clinical evidence available on the use and effectiveness of lumbar interspinous devices and to recommend indications for their use. SUMMARY OF BACKGROUND DATA Lumbar interspinous spacers (ISPs) have recently become popular as an alternative treatment for lumbar degenerative disease. Several spacers are curre...
متن کاملInterspinous process spacers versus traditional decompression for lumbar spinal stenosis: systematic review and meta-analysis.
BACKGROUND Interspinous spacers are used in selected patients for the treatment of lumbar spinal stenosis. The uses of interspinous devices are still debated, with reports of significantly higher reoperation rates and unfavourable cost-effectiveness compared to traditional decompression techniques. METHODS Six electronic databases were searched from their date of inception to December 2015. R...
متن کاملEffects of interspinous spacers on lumbar degenerative disease
The present study aimed to evaluate the early effects of interspinous spacers on lumbar degenerative disease. The clinical outcomes of 23 patients with lumbar degenerative disease, treated using interspinous spacer implantation alone or combined with posterior lumbar fusion, were retrospectively studied and assessed with a visual analogue scale (VAS) and the Oswestry Disability Index (ODI). Pre...
متن کاملCost-effectiveness of three treatment strategies for lumbar spinal stenosis: Conservative care, laminectomy, and the Superion interspinous spacer.
BACKGROUND Lumbar spinal stenosis is a painful and debilitating condition resulting in healthcare costs totaling tens of billions of dollars annually. Initial treatment consists of conservative care modalities such as physical therapy, NSAIDs, opioids, and steroid injections. Patients refractory to these therapies can undergo decompressive surgery, which has good long-term efficacy but is more ...
متن کاملCan a modified interspinous spacer prevent instability in axial rotation and lateral bending? A biomechanical in vitro study resulting in a new idea.
BACKGROUND Interspinous spacers are mainly used to treat lumbar spinal stenosis and facet arthrosis. Biomechanically, they stabilise in extension but do not compensate instability in axial rotation and lateral bending. It would therefore be desirable to have an interspinous spacer available, which provides for more stability also in these two planes. At the same time, the intervertebral disc sh...
